How to Refer - For Providers


Dr. Danny Adel Monsour MD, FRCPC

Dr. Danny Monsour is a neurologist specializing in headache medicine. Referring providers should review the following inclusion and exclusion criteria before submitting a referral.

Inclusion Criteria

  • Ages 12 and up for primary headache disorders (migraine, new daily persistent headache (NDPH), Trigeminal autonomic cephalalgia).

  • Refractory headache disorders unresponsive to standard treatments, secondary headache disorders in those >18 years-old.

  • Patients requiring advanced therapies, including Botox, nerve blocks, and anti-CGRP therapies.

Exclusion Criteria

  • General neurology consultations (e.g., seizures, multiple sclerosis, stroke).

  • Acute headache conditions requiring emergency evaluation, such as thunderclap headache.

  • Post-concussion syndrome when non-headache symptoms are the primary concern.

  • Suspected idiopathic intracranial hypertension (IIH) without confirmed papilledema (patients should be referred to neuro-ophthalmology or the emergency department for further evaluation before referral).

  • Suspected cerebrospinal fluid (CSF) leaks should be referred to the UHN Intracranial Hypotension Program via this referral link: TAPMI External Referral Form.

Dr. Yael Friedman is a neurologist specializing in neuromuscular disorders and general neurology. She provides comprehensive assessments for a range of neurological conditions, including nerve and muscle disorders. Referring providers should review the following inclusion and exclusion criteria before submitting a referral.

Inclusion Criteria

  • Electromyography (EMG) and nerve conduction studies (NCS) for suspected neuromuscular conditions.

  • General neurology consultations.

Exclusion Criteria

  • Acute neurological emergencies (patients should be directed to the emergency department).

  • Suspected stroke or TIA (should be referred to a stroke clinic or emergency department).

Dr. Marissa Lagman-Bartolome is a neurologist specializing in adult and pediatric headache medicine. Referring providers should review the following inclusion and exclusion criteria before submitting a referral.

  • Ages six and up.
